Moundview’s Memory Assessment Clinic is the only memory center within a 70 mile radius of Friendship. Our staff are considered regional experts in the diagnosis and treatment of memory problems. The clinic is affiliated with the Wisconsin Alzheimer’s Institute, a center within UW Medical School’s Department of Medicine. Patients are linked to the latest in memory research, expertise and treatments. Memory loss can occur at any age. It may be triggered by a disease or illness such as Alzheimer’s, stroke, diabetes, chemical imbalance or depression. It may also be caused by head trauma, stress, or medications. Many of these conditions can be successfully treated, especially in the early stages of onset. At the memory clinic, patients complete a memory assessment which is a comprehensive two hour evaluation consisting of a complete medical history, physical, cognitive testing, and patient/family interview. Further laboratory tests, x-rays, and treatment may be recommended to determine the appropriate course of care. Following diagnosis, periodic follow-up evaluations are made to assess the effectiveness of treatments and interventions. Family or significant others may be involved in a patient’s evaluation and treatment if desired. The clinic staff work cooperatively with the patient’s primary healthcare provider. Consultation with the patient’s physician is required to ensure continuity of care. The following forms of payment are accepted: Medicare, Medicaid, self-payment, private insurance and some HMOs (with pre-authorization). Staff is available to assist patients/families in determining payment options. Anyone can make a referral for services. Appointments may be scheduled by calling (608) 339-8360.
