Moundview Home Health provides medical care to patients in the comfort of their homes. Whether you need nursing care, rehabilitation, or assistance with daily tasks, we offer a variety of services to meet your needs. Those who may benefit from services include:

  • Persons recently discharged from a hospital or nursing home
  • Individuals with a chronic condition or disability such as diabetes or stroke
  • Persons needing temporary care while recovering from an accident, heart attack or surgery
  • Persons needing temporary assistance with bathing, dressing, etc.

Skilled Nursing Services: Wound care, home infusion therapy, tube feedings, giving injections, caring for catheters or ostomies, set-up of oral medications, and education on patient care for the patient, family or friends.

Rehabilitative Care: Physical therapy to help patients who need to regain strength, balance, mobility or range of motion. Occupational therapy to help patients improve self-care skills, such as dressing, through therapeutic exercises or adaptive equipment. And, speech therapy to help patients improve impaired speech, swallowing and use of mental skills.

Home Health Aide: Works under the direction of a registered nurse.  Provides assistance with bathing, dressing, oral hygiene, hair, skin, and nail care.  May also assist with simple procedures as an extension of nursing and rehabilitative care, such as helping patients with walking and movement exercises. 

Telehealth: Patients who meet specific criteria may receive in-home telehealth monitoring systems.  First priority is given to individuals with congestive heart failure, diabetes or respiratory conditions as these are conditions that can change on a daily basis.  The telehealth system is connected to medical devices that record the patient's blood pressure, pulse, oxygen level, and weight.  Blood sugar is measured separately and entered into the system by the patient. The patient's health data is transmitted through their telephone line to the home health office.  Staff can access this information on a daily basis, Monday through Friday.  If they see that a patient's condition has changed, they can take corrective action to avoid patient hospitalization or an emergency room visit. 

Lifeline Program: The Lifeline Program is available to elderly and disabled individuals who live alone. Lifeline is a personal response system that links individuals to 24-hour emergency assistance at the touch of a button, should the person fall or have a medical emergency.
